In Tamil Nadu the practical route is the online REV-103 form on the e-Sevai portal: you fill it on screen, attach documents, and pay. A printed form does exist for offline / e-Sevai-centre use, but the online application is faster and is what produces the downloadable digitally signed certificate at the end. So the useful thing to prepare isn't a form — it's your documents.

The field enquiry decides it
An income certificate isn't granted on your uploads alone — a VAO or Revenue Inspector verifies your family's income in the field, and the Tahsildar signs based on that. Remember an income certificate is generally valid one year; see validity and renewal for when you'll need a fresh one.
The offline route, if you'd rather
If you prefer paper, you can complete the income certificate application at an e-Sevai centre, where the operator submits the REV-103 form for you with your documents. This suits people who don't have a CAN or aren't comfortable uploading files. The documents and the VAO enquiry are identical either way — the offline route only changes who types the form. Whichever you choose, insist on the printed acknowledgement with the application number, because that is what lets you track the application and, once approved, download the certificate from a CAN account later. A physical blank form also exists in some offices, but for the vast majority of applicants the online or centre-assisted route is faster and cleaner.
What counts as "family income" on the form
The REV-103 form asks for annual family income — not just the applicant's salary. That means the combined income of the whole family from all sources: salary, business or agricultural income, pension, rent and other regular earnings, usually for the previous financial year. Under-stating it to qualify for a scheme is risky, because the VAO enquiry cross-checks against your visible circumstances; over-stating it can push you above a scholarship's income limit unnecessarily. Enter the true, current figure and make sure it's consistent with whatever proof you attach.
Common form-filling errors to avoid
- Name not matching Aadhaar — copy it character for character, including the position of initials.
- Wrong financial year — use the year the scheme asks for.
- Blurred income proof — a salary slip must be fully readable.
- Wrong taluk — apply where your ration-card address falls.
